Bridgeport
GOALS OF
SERVICES: Catholic Charities Bridgeport seeks to help families
by working to improve the level of emotional and psychological functioning
for individuals, couples, and families. CFS seeks to help mothers
and fathers of unplanned pregnancies with decision making and coordination
of support systems; to provide case management for families dealing
with HIV/AIDS within the Scattered Site Housing Program; to provide
case management for clients of VNS and other contracted providers,
to provide family-based day care; and to provide Parent Education
for court referred clients.
GEOGRAPHIC
AREA SERVED: Greater Bridgeport including Bridgeport, Easton,
Fairfield, Monroe, Stratford, Trumbull, and Shelton.
COUNSELING
SERVICES PROGRAM NARRATIVE: Catholic Charities is an outpatient
counseling program serving individuals, couples, and families living
in Greater Bridgeport. Comprehensive program planning for unplanned
pregnancy couples or individuals is also available. Catholic Charities
provides professionally trained staff to assist families, individuals,
and children; people with marital difficulties, parent/child difficulties,
and individual adjustment problems. Catholic Charities is a Mental
Health Clinic, licensed by the State of Connecticut. Some services
are covered by Medicaid, Medicare, and third party insurance reimbursement.
The counseling services utilize a sliding fee scale, which takes
into consideration the size of the family and the annual income.
Medical Director/Psychiatric Consultant works with the social work
and counseling staff directly regarding patient clinical issues
and medications within the treatment plan. Varied treatment models
are utilized within the therapeutic process in confronting the various
problem areas inclusive of individual, couple, family, and group
process.
ELIGIBILITY:
The programs and services of Catholic Charities are open to all
regardless of religion, race, income, or marital status.
SUPPORTIVE
HOUSING: Provides a network of subsidized rental apartment units
to individuals/families with identified disabilites who without
subsidies for affordable housing, would likely remaing homeless
in the Bridgeport area. Follow-up case management services is also
provided for this population.
PARENT EDUCATION:
Provides a 6 hour seminar to assist divorcing/separating couples
to better understand and respond to the changing needs of their
children.

Stamford
PRIMARY SERVICES:
Mental Health Services; Psychiatric Services; counseling to individuals,
families, couples, and groups. Outreach Pregnancy Services, Right
to Know, Adoption Service within agency jurisdiction, Outreach to
Hispanic Elderly, Families & Children; Trauma Treatment Team, Intensive
Outreach Monitoring Program.
GOALS OF
SERVICES: Improve level of emotional and psychological functioning
for individuals, couples, and families. Assist in authorized adoption
searches. Help mothers and fathers of unplanned pregnancies with
decision making and support system coordination.
GEOGRAPHIC
AREA TO BE SERVED: Darien, Greenwich, and Stamford.
PROGRAM NARRATIVE:
Catholic Charities is an outpatient behavioral mental health program
serving individuals, couples, families, and groups living in the
Greater Stamford area, also aiding adults over 18 with information
regarding adoption when authorized. Comprehensive program planning
for unplanned pregnancy for couples or individuals is also available.

SERVICES: Provides professionally trained staff to assist families,
individuals and children, couples with martial difficulties, parent/child
relationship conflicts, and individual adjustment problems. Catholic
Charities is a Behavioral Mental Health Clinic, licensed by the
State of Connecticut and able to receive Medicaid, Medicare, third
party insurance reimbursement, and managed care arrangements. A
Medical Director/Psychiatric Consultant helps the clinical staff
directly regarding patients, clinical issues, and medications within
the treatment plan. Varied treatment models are utilized within
the therapeutic process in confronting the various problem areas
inclusive of individual, couple, family, and group process.
ELIGIBILITY:
The programs and services of Catholic Charities are open to all
regardless of religion, race, income, or marital status.
HISPANIC
OUTREACH SERVICES TO HISPANIC FAMILIES AND SENIORS: Based at
the Stamford Senior Center. Services involve outreach to Hispanics,
60 years of age and over. Includes development of support network
for participants and for homebound elderly. In addition, we now
offer services to Hispanic families. Case management, translation,
advocacy, assistance with finding housing are some of the services
offered.
PARENT EDUCATION
GROUPS: Divorcing parents referred through a court mandated
program established by Connecticut Legislation effective 1/1/94.
Involves a 6 hour educational program. Purpose is to educate parents
regarding impact of family restructuring upon children.
SENIOR NEIGHBORHOOD
SUPPORT SERVICE: Based at the Quintard Terrace Senior Housing
Facility, this program provides a broad range of case management,
socialization, educational, and advocacy services to low-income
and minority elderly living in Stamford.
OTHER PROGRAMS
INCLUDE: Employee Assistance Program, Crisis Intervention, Support
Groups - includes Women's Issues and other topics as they evolve
Family Life Education, and a Speaker's Bureau.
SPONSOR CONSORTIUM
OF PROVIDERS FOR HEALTHY FAMILIES: Support Network of 35 agencies
within the Greater Stamford Community providing services to low-income
families, pregnant women, and single parents. The purpose of the
network is to identify the needs of this population, to plan for
services, to meet those needs, and to act as advocates for the clients
served.

The programs and services of Catholic Family Services are open to
all regardless of religion, race, income, or marital status.
METHOD OF
PAYMENT: Many of our services are covered by many managed care
companies, Major Medical, Medicaid (Title XIX), Medicare and private
insurance. Services are also available on a sliding scale fee basis.

SERVICES SERVICES PROVIDED FOR UNPLANNED PREGANCY: Involves
counseling for the unmarried mother and her family around the birth
plan for the infant regarding whether the mother will continue to
parent or choose adoption.
SENIOR CITIZEN
OUTREACH PROGRAM: Provides specialized outreach assessment and
psychiatric counseling of the elderly in their own homes or place
of residence.
FAMILY PRESERVATION
PROGRAM: A State of Connecticut Department of Children and Families
funded program which provides intensive case management services
to a small number of at-risk youth and their families on a 24 hour
basis.
PARENT EDUCATION
PROGRAM: Parent Education series, court-approved and required
by Public Act 93-319, for divorcing couples with children.
TELE-CONFERENCE:
Home bound elderly participate in discussions regarding health finance,
reminiscence, and various other therapeutic social activity.
SHARE A SPIRIT
PROGRAM: At Christmas, Share a spirit program provides gifts
and food to the needy children and families in the greater Danbury
area.
WAYS TO
WORK FAMILY LOAN PROGRAM: Finding and keeping a job can be a
challenge for working parents. Occasionally, expenses arise that
a family is not prepared for, such as buying or repairing a car,
paying for child care or buying a uniform or tools needed on the
job.

GEOGRAPHIC
AREA SERVED: Greater Norwalk including Westport, Wilton, Weston
and New Canaan.
PRIMARY SERVICES:
Mental Health Services, Psychiatric Services, Counseling to individuals,
Families and couples; Assists birthparents of unplanned pregnancies
with decision-making and support services; child care services,
Parent Education Program (PEP).
ELIGIBILITY:
The programs and services of Catholic Charities are open to all
regardless of religion, race, income or marital status.
FEE:
Because of funding from the United Way, state grants and charitable
contributions, we are able to provide subsidized rates to those
unable to pay for the full cost of services. Our business office
will help workout an affordable fee.
METHOD OF
PAYMENT: Many of our services are covered by managed care companies,

meet the needs of pre-school children who require a full day of
care while parents work. This center-based component is a state-licensed
facility, which provides on-premises child care by a team of professionals.
